The Procedure
The choice of surgical method depends on the severity of the gynecomastia and the patient’s characteristics. Depending on the case, the surgery can be performed either using liposuction or with subcutaneous mastectomy.
Liposuction: It is used when fatty tissue is the main cause of the problem. Surgery takes approximately 1-1.5 hours and is performed under local anesthesia and sedation, with no need for hospitalization.
Subcutaneous Mastectomy: The mammary gland is removed through an incision around the areola. Surgery takes 1-2 hours and is performed under general anesthesia or sedation. Drainage tubes are often placed to avoid hematoma.
In some cases, the plastic surgeon may combine both techniques or proceed with a simultaneous reduction of the areola and excision of excess skin for a better aesthetic result.
Regarding Gynecomastia
Puffy Nipples
This is a mild form of gynecomastia in which a small amount of mammary gland tissue is located just beneath the nipple and areola. Although the problem is not objectively severe, it causes significant embarrassment and anxiety to the patient. In these cases, the treatment is surgical removal and involves the excision of the glandular tissue through a small crescent-shaped incision at the lower edge of the areola.
Recovery After Gynecomastia Surgery
After the operation, the patient wears a special compression garment for 4 weeks, which helps with proper healing and reduces swelling. Bruising and swelling gradually subside, while mild pain is controlled with simple painkillers.

Surgical Technique
The incisions in gynecomastia surgery are small and strategically placed so that they become almost invisible.
- Liposuction: A few millimetres in length incisions that almost disappear after complete healing.
- Subcutaneous Mastectomy: Incision around the perimeter of the areola, which heals gradually and becomes inconspicuous. Depending on the case, the incision may be crescent-shaped at the lower edge of the areola (KEYHOLE) or circular (PERIAREOLAR) with simultaneous removal of skin.

Is the result of the surgery permanent?
Yes, the removal of the gland and fat is permanent. However, significant fluctuations in weight may affect the result.
