1. Introduction
The office of Gotsikas Ioannis, a plastic surgeon, operates legally according to no. C1 / 4065 license of the Medical Association of Thessaloniki.
In the context of the operation of our practice, we inform you about the following in accordance with article 13 of the General Regulation for the Protection of Personal Data (hereinafter GDPR):

First Name, Last Name, Father’s Name, Gender, Age, Job, Address, Date of Booking, as well as any other essential element associated with the provision of care to you, such as, indicatively, your health problems, the reason for your visit, the treatment followed, etc
3. Purposes and legal bases for processing your personal data:
The provision of your data is necessary both for the performance of the contract between us and for our compliance with our legal obligations. Any refusal or failure to provide us with this data may result in us being unable to perform the contract (full or full). We process the data you provide to us only to the extent necessary for:
(a) the provision to you of our medical services in the context of the execution of the oral contract between us for the provision of medical services, as well as the electronic prescription and referral through EOPYY (art. 14 of N. 3418/2005 and art. 9 par. 2 Ca. GDPR),
(B) our compliance with the obligations under the Code of Medical Ethics (n. 3418/2015),
(c) any foundation, exercise, support and execution of our legal claims, if this becomes necessary (art. 9 par. 2 Ca. (F of the GDPR)

We keep your information in the required period, after which the limitations of legal claims expire in accordance with the applicable law.

Potential recipients of your data are the following:
Our legally authorized staff, public bodies, such as EOPYY, in the case of electronic prescription and referral, for the purpose of our compliance with our legal obligations, cooperating with US IT companies with the purpose of maintaining, repairing our software and/or providing cloud services.
In the event that third parties request that your personal data be transmitted to them in order to establish, exercise or support their legal claims in accordance with Article 9 par.2 Ca. in the GDPR, our practice undertakes to re-inform you of the specific request for the transmission of your data in order to be able to exercise your relevant legal rights.
6. Your rights
In accordance with the General Regulation on the Protection of your Personal Data you are entitled to submit a request for access to your data that we hold (article 15 of the GDPR), request for rectification (article 16 of the GDPR), deletion of your data under certain conditions (article 17 of the GDPR), request for restriction of processing (article 18 of the GDPR), request portability of your data (article 20 of the GDPR), as well as submission of a complaint to Data Protection Authority in case you consider that we have processed your data contrary to the law.
